Direct Support Person - Morton

Position Summary:                      

This position is primarily responsible for the implementation of documentation of consumer programming (i.e. individual formal programming, individual and group activities, incident reports, seizure reports, etc.), providing direct contact and care including but not limited to, assisting with feeding, toileting, repositioning, and transferring needs, and supervising specific in house jobs.

 

Skills/Knowledge/Abilities:

For entry into this position, the individual must have the following skills:  Good communication skills (both written and verbal), reading skills, and math skills at a secondary level.

The individual should be knowledgeable in program implementation, universal precautions, job training, and general safety procedures.

 The individual must have the ability to frequently lift up to 50 pounds from floor to waist high, to teach job training skills and basic adult daily living skills, and to document information factually.

 

Qualifications:

High School Diploma or GED.  Direct Support Person certification and at least one year of experience with the developmentally disabled preferred.

 

Duties and Responsibilities: 

  1. Accurately implement and document all programs based on the individual's formal plan. 
  1. Participate in the development of individual and group activities. 
  1. Adhere to the daily schedule set forth for the program areas ensuring continuous active treatment for each individual is provided. 
  1. Record and document all necessary documentation; including but not limited to, weekly chronological report forms, seizure reports, event communication reports, data sheets, nursing reports, etc. 
  1. Provide Case Managers with pertinent individual information in preparation for the staffing. 
  1. Assist in all necessary ADLs (i.e. toileting, personal hygiene, feeding, repositioning, transfers, etc.) 
  1. Complete payroll/attendance forms as required. 
  1. Participate in assigned door duty to ensure safety is maintained during transportation transfer. 
  1. Complete daily clean up of program areas and common areas of HSC at the end of each day or as needed throughout the day. 
  1. Implement, follow, and monitor all TCRC safety policies and procedures. 
  1. Follow TCRC Communication Model. 
  1. Assist in the orientation and training of new staff and volunteers when appropriate. 
  1. Attend in-service and team meetings as required. 
  1. Serve on assigned committees. 
  1. Other duties as assigned that are not inconsistent with skills, abilities, and qualifications.
