Case Manager

Position Summary: 

This position is responsible for Case Management Services in relation to consumer programming needs.

 

KnowledgeSkillsAbilities:

For entry into this position the individual must have the following skills: Excellent communication skills (both written and verbal) that are professional, clear and concise, reading skills and math skills at a post secondary level.  The individual must have good observation and organizational skills.

The individual should have a working knowledge of the Case Management system and knowledge of developmental disabilities.  Knowledge of the DHS and DHS/DRS program is preferred.

The individual must possess the skills to communicate with adults of varying levels of ability, the ability to document well what is observed, and the ability to complete reports concerning consumer needs and progress in a timely manner.  The individual must have the ability to lift up to 50 pounds from floor to waist high.  This individual must have the ability to plan ahead, multitask, and prioritize in an effective manner.

 

Qualifications:

Bachelor's Degree in Human Service field or related field.  One year of experience working with individuals with developmental disabilities is preferred to meet QIDP status.  This individual must have a valid driver's license and up to date personal automobile insurance coverage (as defined in the TCRC Personnel Policies) as well as meet TCRC's commercial insurer's criteria as an acceptable insured driver.  

 

Duties and Responsibilities: 

  1. Be an active participate in the Interdisciplinary Team. 
  1. Have personal involvement with individuals on caseloads by teaching and instructing Direct Support Person staff how to implement consumer programming to meet their goals and objectives developed in the individual's formal plan, including behavior plans when needed. This includes formal training with DSP on how to implement and document goals and observation of said goals/ behavior plan implementation. 
  1. Maintain direct contact with consumers, parents, guardians, outside agencies, family, other professionals, and intra-agency employees. 
  1. Act as a liaison and advocate for the consumers. 
  1. Prepare and evaluate documentation on monthly, bi-annually, and annual basis and provide direct supervisor with required documentation as requested. 
  1. Review and update consumer files following a set Q.I. schedule. 
  1. Schedule and attend in-services, seminars, conferences, and etc. as required to maintain QIDP status. 
  1. Oversee day to day operation of assigned day training program in conjunction with the Vice President of HSC Day Training. 
  1. Monitor breaks and lunches as scheduled, participate in door duty coverage as scheduled, and when needed, act as a DSP or Job Coach by assisting with the work/training activities of assigned consumer(s) within their environment by providing them with training, guidance, active treatment, and supervision. 
  1. Implement, follow, and monitor all TCRC safety policies and procedures. 
  1. Follow the TCRC Communication Model. 
  1. Assist in the orientation and training of new staff and volunteers when appropriate. 
  2. Attend in-service and team meetings as required. 
  1. Serve on assigned committees. 
  1. Drive as needed in an agency vehicle or own personal vehicle. 
  1. Other duties as assigned that are not inconsistent with skills, abilities, and qualifications.
